VA Aid & Attendance in 2026: The Pension Add-On That Pays for Care

Aid & Attendance is the highest tier of VA pension - and in 2026 the maximum for a veteran with a spouse runs about $33,548 a year. Most families that qualify never apply.

The facts

  • 01Aid & Attendance (A&A) is an enhanced level of VA Pension (or an add-on to DIC for survivors) for wartime-era veterans or survivors who need help with activities of daily living - bathing, dressing, eating - or who are bedridden or in a nursing home.
  • 022026 maximum annual pension rates (MAPR) with A&A: about $28,300 for a veteran alone (~$2,358/mo) and about $33,548 for a veteran with a spouse (~$2,796/mo); a surviving spouse with A&A is about $18,187 (~$1,515/mo). VA pays the difference between countable income and the MAPR.
  • 03For service-connected veterans, the equivalent is Special Monthly Compensation (SMC) under 38 U.S.C. 1114 - paid at higher rates than pension-based A&A (the 2026 SMC-L level is over $4,900/mo).
  • 04Eligibility is means-tested: the 2026 net-worth limit is about $159,240, with a 36-month look-back on asset transfers that can trigger a penalty period.
  • 05You cannot receive pension-based A&A and service-connected compensation at the same time - VA pays whichever is greater, and a service-connected award that beats the pension rate is usually the better outcome.
